      Towards a concept of embodied autonomy: In what ways can a patient’s body contribute to the autonomy of medical decisions?

          Abstract

          “Bodily autonomy” has received significant attention in bioethics, medical ethics, and medical law in terms of the general inviolability of a patient’s bodily sovereignty and the rights of patients to make choices (e.g., reproductive choices) that concern their own body. However, the role of the body in terms of how it can or does contribute to a patient’s capacity for, or exercises of their autonomy in clinical decision-making situations has not been explicitly addressed. The approach to autonomy in this paper is aligned with traditional theories that conceive autonomy in terms of an individual’s capacities for, and exercises of rational reflection. However, at the same time, this paper extends these accounts by arguing that autonomy is, in part, embodied. Specifically, by drawing on phenomenological conceptions of the experience of autonomy, we argue that, in principle, the body is a necessary component of the capacity for autonomy. Secondly, through the presentation of two different cases, we highlight ways in which a patient’s body can contribute to the autonomy of treatment choices. Ultimately, we hope to encourage others to explore additional conditions under which a concept of embodied autonomy should be employed in medical decision making, how its underlying principles might be operationalised in clinical situations, and its consequences for approaches to patient autonomy in healthcare practice, policy, and law.

          Supersizing the Mind : Embodiment, Action, and Cognitive Extension

          Studies of mind, thought, and reason have tended to marginalize the role of bodily form, real-world action, and environmental backdrop. In recent years, both in philosophy and cognitive science, this tendency has been identified and, increasingly, resisted. The result is a plethora of work on what has become known as embodied, situated, distributed, and even ‘extended’ cognition. Work in this new, loosely-knit field depicts thought and reason as in some way inextricably tied to the details of our gross bodily form, our habits of action and intervention, and the enabling web of social, cultural, and technological scaffolding in which we live, move, learn, and think. But exactly what kind of link is at issue? And what difference might such a link or links make to our best philosophical, psychological, and computational models of thought and reason? These are among the large unsolved problems in this increasingly popular field. This book offers both a tour of the emerging landscape, and an argument in favour of one approach to the key issues. That approach combines the use of representational, computational, and information-theoretic tools with an appreciation of the importance of context, timing, biomechanics, and dynamics. More controversially, it depicts some coalitions of biological and non-biological resources as the extended cognitive circuitry of individual minds.

            When the world becomes 'too real': a Bayesian explanation of autistic perception.

            Perceptual experience is influenced both by incoming sensory information and prior knowledge about the world, a concept recently formalised within Bayesian decision theory. We propose that Bayesian models can be applied to autism - a neurodevelopmental condition with atypicalities in sensation and perception - to pinpoint fundamental differences in perceptual mechanisms. We suggest specifically that attenuated Bayesian priors - 'hypo-priors' - may be responsible for the unique perceptual experience of autistic people, leading to a tendency to perceive the world more accurately rather than modulated by prior experience. In this account, we consider how hypo-priors might explain key features of autism - the broad range of sensory and other non-social atypicalities--in addition to the phenomenological differences in autistic perception.               Intellectual disability and its relationship to autism spectrum disorders.

              Intellectual disability (ID) and autism spectrum disorders (ASDs) covary at very high rates. Similarly, greater severity of one of these two disorders appears to have effects on the other disorder on a host of factors. A good deal of research has appeared on the topic with respect to nosology, prevalence, adaptive functioning, challenging behaviors, and comorbid psychopathology. The purpose of this paper was to provide a critical review and status report on the research published on these topics. Current status and future directions for better understanding these two covarying disorders was reviewed along with a discussion of relevant strengths and weaknesses of the current body of research.
