          Zusammenfassung

          Ziel der Studie Wenngleich sich die MBOR insbesondere im Bereich der Muskel-Skelett-Erkrankungen längst als Bestandteil des rehabilitativen Versorgungssystems etabliert hat, deuteten auf Routinedaten basierende Studien auf eine ausgeprägte Heterogenität hin, was die Umsetzung des MBOR-Anforderungsprofils betrifft. Im Rahmen der vorliegenden Untersuchung wurde die Realisation des Arbeitsplatztrainings sowie auch der vorgeschalteten Leistungsdiagnostik nun erstmalig auf Grundlage einer bundesweiten Befragung von Akteurinnen und Akteuren der MBOR in den Blick genommen.

          Methodik 2023 erfolgte eine als Vollerhebung angelegte schriftliche Befragung von Rehabilitationseinrichtungen mit Zulassung für orthopädische MBOR. Zum Einsatz kam ein in Anlehnung an das MBOR-Anforderungsprofil entwickeltes Instrument. 86 der 317 angeschriebenen Einrichtungen beantworteten den Fragebogen und erfüllten zugleich die Einschlusskriterien (Durchführung von MBOR-Heilverfahren).

          Ergebnisse 67% der Fachabteilungen schilderten, zu Reha-Beginn bei mindestens 50% der Personen Verfahren der Functional Capacity Evaluation (FCE) einzusetzen und 70% die regelhafte Durchführung spezifischer Profilvergleiche. Ferner berichteten 57%, das Arbeitsplatztraining auf Grundlage eines Profilvergleiches zu gestalten. Das Arbeitsplatztraining selbst wird bei etwa jeder zweiten Fachabteilung bei mindestens 50% der Personen im Umfang von mindestens 360 Minuten erbracht. Die Durchführung der FCE-Testungen wie auch des Arbeitsplatztrainings erfolgt dabei vorrangig durch Ergotherapeutinnen und Ergotherapeuten.

          Schlussfolgerung Leistungsdiagnostik und Arbeitsplatztraining werden vielfach, jedoch längst noch nicht flächendeckend in Einklang mit dem MBOR-Anforderungsprofil realisiert. Ein vielversprechender Ansatzpunkt zur einrichtungsinternen Weiterentwicklung und Optimierung der MBOR liegt in der Förderung der Interdisziplinarität.

          Abstract

          Purpose Although work-related medical rehabilitation (WMR) has been established as a component of the rehabilitative care system for some time, particularly in patients with musculoskeletal disorders, studies indicated strong heterogeneity regarding the implementation of the guideline for WMR in Germany. This study is the first to examine the implementation of work-related functional capacity training and the preceding work-related diagnostics based on a nationwide survey of WMR health care professionals.

          Methods Rehabilitation facilities (n=317) authorized to carry out WMR in patients with musculoskeletal disorders were invited to participate in a nationwide survey. 86 of 317 facilities responded to the self-developed questionnaire and met the inclusion criteria.

          Results 67% of the WMR facilities described a systematic use of functional capacity evaluation (FCE) at the beginning of rehabilitation in at least 50% of the rehabilitants. 70% stated that they systematically matched abilities and work demands. Furthermore, 57% reported that profile comparisons were used to tailor training of work-related functional capacity. The training itself was provided to at least every second rehabilitant with a total duration of 360 minutes or more in about every second facility. The FCE procedures and the work-related functional capacity training were primarily carried out by occupational therapists.

          Conclusion Work-related diagnostics and the work-related functional capacity training are often but not yet always implemented in line with the guideline for WMR. Promoting interdisciplinarity seems to be a promising approach regarding the further development of WMR.

          The Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) statement: guidelines for reporting observational studies.

          Much of biomedical research is observational. The reporting of such research is often inadequate, which hampers the assessment of its strengths and weaknesses and of a study's generalizability. The Strengthening the Reporting of Observational Studies in Epidemiology (STROBE) Initiative developed recommendations on what should be included in an accurate and complete report of an observational study. We defined the scope of the recommendations to cover three main study designs: cohort, case-control, and cross-sectional studies. We convened a 2-day workshop in September 2004, with methodologists, researchers, and journal editors to draft a checklist of items. This list was subsequently revised during several meetings of the coordinating group and in e-mail discussions with the larger group of STROBE contributors, taking into account empirical evidence and methodological considerations. The workshop and the subsequent iterative process of consultation and revision resulted in a checklist of 22 items (the STROBE Statement) that relate to the title, abstract, introduction, methods, results, and discussion sections of articles. Eighteen items are common to all three study designs and four are specific for cohort, case-control, or cross-sectional studies. A detailed Explanation and Elaboration document is published separately and is freely available on the web sites of PLoS Medicine, Annals of Internal Medicine, and Epidemiology. We hope that the STROBE Statement will contribute to improving the quality of reporting of observational studies.

            Effects of nationwide implementation of work-related medical rehabilitation in Germany: propensity score matched analysis.

            Since 2014, the Federal German Pension Insurance has approved several departments to implement work-related medical rehabilitation programmes across Germany. Our cohort study was launched to assess the effects of work-related medical rehabilitation under real-life conditions.

              A critical review of functional capacity evaluations.

              The role of functional capacity evaluations (FCEs) appears to be increasing as employers and insurers rely more heavily on them for decision making. To meet credibility requirements, and the American Physical Therapy Association's standards for measurement and documentation, all FCE suppliers need to validate and refine their systems. This article provides information that can be used to make informed decisions in the selection of an FCE and in functional assessment practices. Features of well-designed FCEs are discussed. Ten well-known FCE systems are analyzed according to these features and other common characteristics. Current issues such as the qualifications of the evaluators, reliability and validity, length of assessments, projection of endurance to 8-hour workdays, standards of practice, safety protocols, and behavioral assessment and management strategies are discussed.
